Wow. Ive watched the Matrix a lot and even I didn't notice some of those. I would like to point out though that you missed one I did spot. The bullet time scene on the roof, the sky changes from sunny to cloudy to sunny again. As Neo dodges the bullets he has a shadow, when he falls he has no shadow and when the agent is about to shoot him he has a shadow again. Just another one to put in there.
Great thing about the film is that the goofs could simply be glitches in the Matrix :0

If you watch the movie again , you'll see when the agent takes over the helicopter's pilots body , he comes out looking sideways while the pilot was looking forward.. when he gets shot at the temple the pilot has got a bullet right between his eyes just because of that !!!
that was one of the things taht I noticed and really liked about Matrix.. but good job on the other inconsistencies !!

This is the matrix, the world seems to be ordinary at first, then neo slowly discovers this isent so, so dont u awl think these inconsistencies could add to the movie if u view them this way.
when agent smith punches a hole in the wall with fist, then not long later the wall is fully formed ( the reason this happens is because of the matrix) this could add to the surrealism of the movie if viewed that way and not as a mistake.
all other scenes this guy comments can b viewed same way
